What We Like

  • 40% zinc oxide is the strongest OTC option for clearing up bad rashes
  • Creates a thick moisture barrier that lets irritated skin heal
  • Most rashes start improving within 12-24 hours
  • Pediatrician recommended, which gives you confidence it actually works
  • Affordable and available everywhere, no hunting for specialty products

What Could Be Better

  • Very thick and pasty, gets under your fingernails and is hard to wash off
  • Will stain cloth diapers (use disposable liners if you cloth diaper)
  • Too heavy for everyday preventive use, save it for active rashes

Safety Notes

For external use only. Avoid contact with eyes. If rash doesn't improve after 7 days of use, consult your pediatrician. Keep tube sealed when not in use. If rash has blisters or open sores, see a doctor before applying.

Buying Guide Notes

Keep Desitin Maximum Strength for active rashes and use a lighter cream daily. Aquaphor Baby is our go-to for everyday prevention. For a natural alternative, Earth Mama makes an organic diaper balm that works for mild rashes. If rash persists or looks yeasty (bright red with satellite dots), call your pediatrician since you might need an antifungal.

Full Review

There are two camps in the diaper cream world: the everyday preventive creams and the heavy-duty rescue creams. Desitin Maximum Strength is firmly in the rescue camp. At 40% zinc oxide, this is the strongest over-the-counter diaper cream you can get without a prescription. When your baby gets a bad diaper rash (and they will, especially during teething or when starting solids), this is what you reach for. It creates a thick, paste-like barrier that seals out moisture and lets the skin heal underneath. Most rashes start improving within 12-24 hours. Is it thick and pasty? Yes. Will it get under your fingernails and stain cloth diapers? Also yes. But when your baby is screaming during diaper changes because their bottom is raw, you don't care about any of that. You just want it to work. And it works. The pediatrician recommended this one specifically when our son had a nasty yeast-related rash that wouldn't clear up with gentler creams. Two days of Desitin Maximum and it was nearly gone. For everyday prevention, I'd use something lighter like Aquaphor or a regular zinc cream. Save this one for when things get bad. Because they will get bad at least a few times, and you'll be grateful you have a tube ready.
